Full Time ​RN Case Manager Hospice

Job Description

Join our team today!! Hospice For All Seasons is one of Central Pa’s leading providers of end of life care. Headquartered right here in Central PA. Our agency cares for patients in a variety of settings throughout Central PA. At Hospice For All Seasons we believe that care should be patient-centered. We believe that hospice is a gift and we work around the clock to provide compassionate and attentive care to every person touched by our program. Currently, we have an opening for a Full-Time Hospice RN. Hospice RN is the case manager of a multidisciplinary team who provide exceptional care for patients during the end of life process. Some of the duties of this position are listed below:

Assumes primary responsibility for a patient/family caseload that includes, assessing, planning, implementing and evaluating phases of the nursing process.
Provides comprehensive nursing assessments as directed by the interdisciplinary team
Participates in developing the interdisciplinary team plan of care
Supervises Licensed Practical Nurses and Certified Nursing Assistants
Maintains up-to-date patient records using computerized medical record system so that problems, plans, actions and goals are accurately and clearly stated and changes are reflected as they occur; documentation is completed to meet regulatory requirements.
Provides completed documentation within 24 hours of starting visit and according to company policy.
Attends Interdisciplinary Team meetings and facilitates discussion of issues from caseload for full team discussion, consultation and evaluation.
Supervises and documents observance of the home health aide plan of care every month.
Assures that home health aide follows documented Home Health Aide Plan of Care.

Required Skills

Ability to travel throughout a multi-county service area
Exceptional Customer Service Skills
Excellent Assessment and Documentation Skills including EMR

Required Experience

License to practice as a Registered Nurse in the PA.
Current PA license and valid automobile insurance.
Has reliable transportation and willing to drive in all kinds of weather.

Part Time RN/LPN On Call  Weekends Overnight 

      Friday 4:30 pm until Saturday 8:00 am

      Saturday 8:00 pm until Sunday 8:00 am

      Sunday 8:00 pm until Monday 8:00 am


Hospice For All Seasons is locally owned and operated hospice provider. At Hospice For All Seasons we believe that care should be patient-centered. We believe that hospice is a gift and we work around the clock to provide compassionate and attentive care to every person touched by our program.

The primary goal of the On-Call nurse is to implement the overall plan of care for each patient in conjunction with the attending physician and case manager, after normal business hours of operation. Responsibilities will include admissions, assessments and follow up visits. The ideal candidate will be able to function independently without supervision and be capable of making decisions when necessary. He/she will have excellent communication skills (both written and verbal), possess excellent nursing judgement and be able to travel to visit patients when necessary.

QUALIFICATIONS

Graduate of an accredited school of nursing.
Valid license as a RN/LPN Nurse in the State of PA
Home Health and/or Hospice experience highly desirable
Minimum one year recent professional nursing experience
Valid PA Driver’s License/Auto Insurance
Able to cope with emotional stress and be tolerant of individual lifestyles
Sensitive to the needs of terminally ill patients & families & one's own feelings about dying & death.
Basic computer skills desirable
